At Trust Risk Management Services, Inc., it is our mission to
make the application process as user friendly as possible. To help ensure a
favorable experience, please make note of the information below prior to
completing the online application.
Please have the following materials available for quick reference:
A copy of your prior professional liability insurance policy (if applicable)
Details of any previous claims history (if applicable)
Names and addresses of any third parties (additional insureds or entities)
certificates/memorandums of insurance.
A credit / debit card for premium payment (Visa or MasterCard)
Important notes on completing the application:
- Mandatory Field Entry - Most of the
fields on the application require a response. You will be prompted to complete
any required fields, and the system will not allow you to move forward through
the remainder of the application unless completed. If there is a missing or
invalid entry and you attempt to proceed, an "errors occurred" box will appear
on the right hand side of the application to guide you to the area of the
application you will need to review.
- Effective Dates - Desired effective
dates must be no earlier than the day following the application date and no
later than 90 days following the application date.
- Selection of Payment Method - Following
receipt of a premium indication, you will be prompted to select a payment
method. The available options are credit / debit card (Visa or MasterCard), or
payment by mail. Once you have chosen a payment method, you will not be allowed
to return to previous screens.
